Aloha!  We appreciate your interest in adopting an animal from the Oahu SPCA.  Our animals have been rescued from living on the streets and are in desperate need of the extra special care and love that can be given to them by you as a Adoptive parent.  This form and a consultation with an Oahu SPCA representative are designed to help you find the dog or cat most compatible with your lifestyle.

To be considered as an Adopter, you must:
– Be 21 years of age or older
– Have identification showing your present address
– Have the knowledge and consent of your landlord
– Be able and willing to spend the time and money necessary to provide medical treatment and proper care for your animal.

Please note that Completion of this form does not guarantee acceptance as an adoptive home / parent. After submitting this form to the Oahu SPCA, a representative will consult with you. This is necessary in order to match the right animal to you for adopting. So, please fill out the following information as completely as possible.

Adoption Application

  • Please double check the spelling of your email address ( Please check your spam folders if you do not receive a message from us within a few minutes.
  • We never make unsolicited calls or sell your phone number!
  • Someone other than you that we can contact in case of emergency.
  • If not a home phone, another phone we could use to get a hold of you.
  • If you answered rent, may we contact the owner to obtain permission for this animal to live in your home? What is the owners name and address?
  • If you have, when did you apply and what was the outcome?
  • If your answer is yes, please explain the circumstances.

  • Do you live in a house, yurt, apartment, house-boat, condo, igloo, town-home, mobile home, mansion or spooky castle? Do you live in the city or the country? Please describe where your adopted pet will be residing.
  • If the answer is yes, please describe the stairs, how many, how high, are they enclosed, etc.
  • Example: "No kids" or "3 kids: 5 year old, 10 year old, 14 year old"
  • If yes, please describe what type of allergy and what you will do to make adopting work with this person's allergy.
    Fostering means you will give a temporary home to an animal until it's adopted. Fostering-to-Adopt means you will foster an animal with the intent of adopting.
  • If you already know which animal you would like to adopt, please let us know it's name, type, location, etc. and we will try and accommodate your wishes. Please be as specific as you can. As an example: "Kiki, the grey striped 4 year old cat from Kapolei shelter, saw him when you were at petsmart on June 8th"
  • Please include how long you had them, ages, sex, breeds and what happened to them.
  • Please include any and all other animals who will be living in the same household as the adopted pet including how long you've had them, ages, sex, breeds, etc.
  • Prior to & during the foster period, are you willing to have an Oahu SPCA representative come to view where your foster animal will live? (You will be given 24 hours notice)
  • If you are not adopting a dog or puppy, please type NA (Not Applicable)
  • If yes, may we contact your vet? What is your vet's name and contact details?
  • If student, where are you attending? If military, please list your commanding officer and contact info.
  • Please enter their name, phone number, email address, location and relationship to you.
  • Remember, questions with an asterix * are mandatory, if you missed any, those questions will be highlighted with red. They must be filled out before you can submit the document.
